Travel forms are to be completed after selection for a program
and prior to travel, typically due by the Spring Chapter Meeting. These forms are now housed on myCISV. You will find more information about your program there as well. If you need help with myCISV please refer to the myCISV FAQ Page, the step-by-step help notes, or this helpful video.

Leader Application Instructions

  1. Pay a $30 application fee to “CISV Smoky Mountain Chapter” on Paypal or email smokymtncisv.finance@gmail.com
  2. Chapter Membership.  Join our chapter or Renew your existing membership.  All leader applicants must have a membership valid through the year of travel. The membership and application fee payments can be made online. We accept payments through Paypal.
  3. Download the Adult Leader Application Packet.
  4. Complete the following forms:
    • Staff-Leader Application Form
    • 2 Confidential Reference Forms for CISV Leadership or Staff Position
    • Background Check
    • Media Release Form
  5. References – Ensure TWO Confidential Reference Form for CISV Leadership/Staff Position are submitted.  Only adults who know the applicant well should submit references. Reference forms must be received by January 6.
  6. Applicants will submit all application materials using this Google Form. Please save / scan each individual piece of the application as a separate PDF prior to beginning the Google Form as you will be required to upload each piece separately.

Village Application Instructions

  4. Download the Youth Meeting Application Packet. If you are applying for the Winter 2024 Youth Meeting, use this Youth Meeting Application Packet. Deadline to apply is ASAP for winter 2024 program and January 6 for summer 2025 programs.

  7. Applicants will submit all application materials using this Google Form for 2025 programs and this Google Form for the 2024 Youth Meeting in Stockholm, Sweden. Please save / scan each individual piece of the application as a separate PDF prior to beginning the Google Form as you will be required to upload each piece separately.

Junior Counselor Application Instructions

  6. Include these supporting documents:
      • Proof of Age (copy of driver’s license, birth certificate or passport)
  7. Email all documents to National Seminar Camp Chair, Julia Beasley at seminar@cisvusa.org, Laura Ripberger the CISV USA Executive Director at cisvusa@cisvusa.org, and our chapter’s VP of Programs at smokymtncisv.program@gmail.com.     Application Deadlines –
    The National Office must receive completed applications by January 1, 2025.  Late applications may be accepted. However, those received by the deadline will receive priority.  For questions or concerns, please contact the National Seminar Camp Chair, Julia Beasley at seminar@cisvusa.org or the Executive Director, Laura Ripberger at cisvusa@cisvusa.org.

Domestic Program Instructions

    • Youth Legal Form – Domestic Activities and Programs (Youth Only)- This form shall be used for all activities occurring in the United States (mini camps, Annual Meeting & Conference/CISV In Motion, National Youth Meeting, National Leadership Training, etc.), by youth under the age of 18.
    • Adult Legal Form – Domestic Activities and Programs (Adult Only)- This form shall be used for all activities occurring in the United States (mini camps, Annual Meeting & Conference/CISV In Motion, National Youth Meeting, National Leadership Training, etc.), by those who are 18 years of age or older.
    • Media Release Form
    • Complete any registration prior to the activity or program
    • Covid Waiver
